Can a Financial Advisor say he will only handle your account if it is Discretionary or Managed? In other words, the client must sign a form indicating you accept this.

Yes. Providing he/she has the proper qualification and licensing to do so.
Many advisors limited their practices to a specific type or segment of the market.
Is the advisor in question doing the discretionary investing decisions himself, or does he put you into a managed portfolio with a professional third party portfolio manager.
As George said, perhaps look for another advisor unless this is what you are looking for.
